What is the C.M.H. Program?

The Complex Medical Help program supports diagnostic and coordinated treatment services for children with chronic health care needs.

To Qualify a Child Must:

• Be under 25 years of age

• Be an Ohio resident

• Be under the care of a CMH approved physician

• Have an eligible medical condition that requires further treatment

• Be financially eligible for CMH in order to consider coverage for treatment services
