ATTENTION PHASE 1B COVID-19 Vaccine Registrants: Once you've completed your registration form (using our online form OR paper version), you WILL receive a call from a 1-800 number with a recorded message from one of our staff members using a system called One Call Now.

It is very important that you answer this call, listen to and follow the prompts to schedule your vaccine appointment.
We have a limited staff and have received over 900 registration forms for Phase 1B.

We're not certain how many doses of the vaccine we will receive each week so we are scheduling accordingly. Please sit tight and know that we are working diligently to get everyone scheduled.
